Jay has thrown his back out two days before an on-site interview abroad and sees a doctor at a walk-in clinic. Before he accepts a prescription he wants to know whether the painkiller will make him drowsy or show up on anything.
Why this conversation
A narcotic is, strictly, a drug that dulls the senses and induces sleep, and in everyday and legal English it means a controlled, addictive drug: heroin, cocaine, or opioid painkillers such as codeine and oxycodone. As an adjective it marks that class ("a narcotic painkiller"). In a clinic it is a practical question about drowsiness, driving, and dependence, not a moral one. Do not stretch it to any strong medicine: ibuprofen or a muscle relaxant is not a narcotic, and a doctor will correct you.
Dialogue
Ken: It's a muscle strain, nothing structural. I'll give you something for the pain and a muscle relaxant for the nights.
Jay: Is the painkiller a narcotic? I have an interview on Thursday and I can't afford to be drowsy or foggy.
Ken: No, it's a high-dose anti-inflammatory. The relaxant will make you sleepy, so take it only at bedtime.
Jay: Good. If it were a narcotic I'd rather manage with heat and stretching until after Thursday.
Ken: Sensible. Sit less, walk more, and don't lift your suitcase with your back.

"Is the painkiller a narcotic? I have an interview on Thursday…"
"If it were a narcotic I'd rather manage with…"
